Release of Liability for Participants

IN CONSIDERATION OF their child/athlete being allowed to participate in JLC Regional Committees and related events, the undersigned acknowledges and agrees that:

• Participation involves the risk of injury, disability, and death, even with proper rules and equipment.
• I, for myself, spouse, and child, knowingly and freely assume all such risks, known or unknown, even if arising from negligence, and take full responsibility for my child’s participation.
• If I observe any concern in my child’s readiness or in the program, I will remove my child and alert an official immediately.
• I release and hold harmless JLC, NC Hub committee, its officers, officials, volunteers, and agents from any liability related to injury, disability, death, or property damage from my child’s participation.
• I indemnify and hold harmless the above parties from any and all liability, even if arising from their negligence.
• I acknowledge the risk of compressed game schedules, especially during long weekends, where children may play consecutive games with little rest.
• I understand that injuries may arise from inclement weather, equipment, poor conditioning, or conduct inherent to youth sports.

Waiver/Release for Communicable Diseases (including COVID-19)

• Participation includes exposure to diseases like MRSA, influenza, and COVID-19. Risk of illness or death exists.
• I freely assume these risks for myself and my child, even if arising from negligence, and accept full responsibility.
• I agree to follow all participation rules related to disease prevention. If I notice hazards, I will withdraw and inform officials.
• I release and hold harmless JLC, its coaches, team managers, volunteers, agents, and sponsors from any claims related to illness or disease, even if due to negligence.
• I understand that all participants must carry their own medical insurance. JLC is not responsible for medical incidents.

Photo/Video Authorization

I consent to the use of photos/videos of me or my child by JLC Regional Committee for brochures, websites, and promotions. I understand these remain the property of JLC. Scorecards and photos may be used for marketing.

For Participants of Minority Age (Under Age 18)

I certify that I, as the parent/guardian, have read and explained this waiver to my child, including their responsibility for safety and rule adherence. My child understands and accepts the risks and terms. I consent and agree to the terms on behalf of my child and our family, even if arising from negligence.
